It is the intent of the Brantford Brant Norfolk Ontario Health Team (BBN OHT) to build trust with the people it serves, and to include them as equal partners in decision-making. In order to meaningfully do that, we must allow people with lived experience (this means you!) to express their ideas and feedback.

With this in mind, we created the BBN OHT Health Navigator. This app provides patients and caregivers with 24/7 navigation of local and provincial health and social services, as well as access to various patient portals in our region. In order to ensure we are providing a valuable experience for the user, we need to hear from you.
