We are creating a tool to support people after they leave the hospital, especially if they came to the Emergency Department for mental health or substance use concerns. What is this tool? It’s a simple sheet of paper that patients get when they go home. It explains: What to do next, Follow-up appointments, How to take medications safely, Community supports and services that can help Why your voice matters: We want to hear from people with lived experience—whether you have personally experienced mental health or substance use challenges, or have supported someone who has. Your feedback helps us understand what would make this tool easy to use, clear, and truly helpful. Your ideas will help shape the final version of the tool, which will be tested in a hospital setting. You can help make this tool better for others in our community!
