Care Options for Residents Without a Family Doctor or Nurse Practitioner

The Brantford, Brant, and Norfolk Ontario Health Team (BBN OHT) Unattached Access Clinics were developed through a partnership between the Norfolk Family Health Team in Simcoe, Primacare in Brant, and the Grand River Community Health Centre in Brantford, and was made possible through funding from the Brantford, Brant, Norfolk Ontario Health Team (BBN OHT). The goal of this initiative is to provide an access point to primary care for individuals who do not currently have a family doctor or nurse practitioner.

 

 

 

Are you a resident of Brantford, Brant, or Norfolk without a Family Doctor or Nurse Practitioner?

Three new temporary Access Care Clinic locations have been established to support individuals in our community who do not currently have a primary care provider.

Who Can Access This Clinic?

We provide care for patients who:

  • Live or work in Brantford, Brant, or Norfolk
  • Do not have a Family Doctor or Nurse Practitioner

Important ** The BBN OHT Community Access Clinics offer scheduled, pre-booked appointments for non-emergency health concerns.

This is not an urgent care or emergency clinic. If you are having an emergency, please dial 911

This is not a walk-in clinic. Appointments must be booked in advance.

Please bring your Health Card, a list of current medications, and any relevant medical documentation (e.g., past test results or specialist notes).

Care is provided by Medical provider (Either a medical doctor or nurse practitioner.)

No. This clinic does not assign permanent providers. However, we can help you register with Health Care Connect to be matched with a Family Doctor or Nurse Practitioner when space becomes available.

We are working to secure long-term funding and remain committed to finding sustainable solutions for primary care in our community.

The clinic serves all ages who do not have a primary care provider.
