The Brantford Brant Norfolk Primary Care Network (PCN)
The Brantford Brant Norfolk Primary Care Network is a largely self-organized group that has been actively engaged as a Brantford Brant Norfolk Ontario Health Team partner since 2019. The Network has been remarkably successful in providing leadership and a cohesive voice for primary care providers in the region.
The group is currently comprised of representatives of the majority of primary care organizations within Brantford Brant and Norfolk, which are affiliated with approximately 101 Nurse Practitioners and Physicians providing care to a patient population of over 200,000. Members have volunteered their time to contribute to many of the Ontario Health Team’s working groups and its overall leadership structure, and have leveraged a collective sense of purpose to spearhead the community’s response to COVID-19.


Current Work

The Brantford Brant Norfolk Primary Care Council (BBNPCC) 2025–2030 Strategic Plan ( Now Primary Care Network)
builds on the foundation of the 2022–2025 plan and reflects a refreshed, provider-led vision for strengthening primary care across Brantford, Brant, and Norfolk. Developed in collaboration with local primary care providers, community partners, and people with lived experience, this plan outlines a shared direction for creating a more connected, equitable, and interdisciplinary primary care system.
The plan introduces five strategic directions that will guide the next five years of regional primary care planning:
- Support Primary Care Providers
- Strengthen Primary Care Leadership
- Advance Digital Innovation
- Expand Equitable Access to Primary Care
- Enhance System Navigation and Integration

Primary Care Provider Clinical Opportunities
The BBNPCN will maintain a database of clinical opportunities for primary care providers in Brantford, Brant and Norfolk.
Clinical opportunities will be submitted to the BBNPCN Executive for review. The BBNPCN Executive will use the following criteria when evaluating potential listings for inclusion.
The clinical opportunity MUST:
- Seek Family Physicians or Nurse Practitioners for the position;
- Provide publicly-funded healthcare services within BBN;
- Align with the BBNPCN Terms of Reference, and Support the BBNPCN Strategic Plan.

The Primary Care Networks in Action Summit
The Brantford Brant Norfolk Primary Care Network (PCN) Summit on March 31 brought together an incredibly engaged group of family physicians, nurse practitioners, and primary care leaders from across the region, all committed to building a more connected, collaborative system of care. The strong participation reflected a shared momentum toward improving patient experiences while also supporting providers—particularly through exploring team-based care and digital solutions that can help reduce administrative burden and burnout. The evening created space for meaningful dialogue, knowledge-sharing, and forward-thinking ideas that will continue to shape primary care in our community. A sincere thank you to our keynote speakers for their insight and leadership, and to everyone who contributed to making this inaugural summit such a valuable and inspiring event.
